Library Card Requirements

1. To receive a new patron card -
   
Print out and complete the library card form from the links below and bring it to the library, or come into the library and fill out the library card form and don't forget to bring your photo ID with you.
2. To renew your patron card  -

Print out and complete or come into the library and fill out the library card form.
3. Replacement card
-
Print out and complete or come into the library and fill out the library card form.
It is $.50 for a replacement card
4. Special note about the children's cards - these forms must be filled out by a parent or guardian at the library. These cards are for ages 18 and younger.
